In consideration of the right for me/my child to participate in JEWISH FAMILY EXPERIENCE/ NCSY (hereafter referred to as JFE/NCSY-OU) events, I hereby agree as follows:

I am aware that I will be held responsible for any damage to public or private property that JFE/NCSY-OU states that I/my child caused and agree to fully reimburse all parties involved.

JFE/NCSY-OU may provide over-the-counter medication ( Benadryl,Advil,Tylenol etc.) as deemed necessary. I understand that this does not require JFE/NCSY-OU to provide medical treatment. I have advised JFE/NCSY-OU of any over the counter medications which may not be administered to myself/ my child. I certify that I/my child are fully capable of participating in all activities associated with this event, and that I/my child has no unreported difficulties that would restrict full participation. I understand that in case of emergency, every reasonable effort will be made to contact me or my emergency contact. If we cannot be reached, I give permission to the physician or EMT selected by JFE/NCSY-OU to hospitalize, secure proper treatment for, and to order injection, anesthesia, or surgery for myself/my child. I agree to reimburse immediately and/or accept primary financial responsibility for the total cost of all medical care provided to myself/my child. By checking below, I release said medical personnel, JFE/NCSY-OU and their respective officers, directors, trustees, affiliates, agents, employees, independent contractors, consultants, advisors, vendors, volunteers, licensees and assignees (collectively “Releasees” and each a “Releasee”) from and against every claim, loss, damage and liability or responsibility, including, without limitation, reasonable attorneys’ fees and costs (“Liabilities”) arising from any judgments or decisions made in the obtaining and rendering of medical assistance and treatment for myself/my child.

I acknowledge and am willing to assume and accept any risks associated with my/my child's participation in any aspect of any JFE/NCSY-OU event, and I agree that the terms of this waiver will likewise bind me, my child, my heirs, legal representatives, and assignees. I release and will defend, indemnify, and hold harmless the JFE/NCSY-OU, its directors, owners, agents, employees, and volunteers ("releasees") from every claim and any liability that I or my child may allege against the releasees (including reasonable legal fees and costs) as a direct or indirect result of harm to me/my child while I/s/he is in the care of JFE/NCSY-OU.

From time to time, JFE, JSU, NCSY and/or the Orthodox Union and its affiliates, employees and agents (together, “JFE”) take photographs and videos of individuals, including minors, who attend JFE events, programs, and other offerings. JFE engages in these activities for purposes relating to the advancement of the religious, spiritual, charitable, and educational goals and objectives of JFE, including but not limited to the sharing of photos/videos in newsletters, on our blogs, and on our website and social media pages for the purpose of keeping our members and others informed of JFE’s events and other activities. Please complete this form to give us permission to take photos/videos of you and/or your children when you participate in JFE events and to use those photos/videos in our printed and online publicity:

1. I hereby authorize JFE to take photographs and/or video of me (or my children) when I (or my children) participate at JFE events. I grant JFE full rights to use and share the images resulting from the photographs and/or videos and any reproductions thereof (the “Recording”), in whole or in part, along with my (or my children’s) name, biographical information and quotations for any purpose including fundraising, publicity, advertising, or on social media, all without further obligation or compensation. I hereby waive any right of review or approval regarding the Recording and JFE shall have the right to edit or delete the Recording in its sole discretion. I understand that all rights in and to the Recording shall be JFE’s sole and absolute property. I understand that if I access JFE’s services through a third party platform, product, service, or application (a “Third Party Service”), content and related information generated while accessing JFE’s services in this manner may be monitored, captured, and or recorded by the third parties that provide such Third Party Services. I consent to the use and sharing of personal information by any such third party as described in the privacy policy available through the Third Party Service. I release and forever discharge JFE from all causes of action, suits, or liability of any kind arising from or in connection with the use or publication of any Recording of me or my children made in connection with my participation in JFE events. I waive any right of privacy associated with the Recording and I waive any right to compensation related to the use of the Recording.
I HAVE READ THIS AGREEMENT AND AM FULLY AWARE OF ITS CONTENTS, INCLUDING THE RELEASE THAT I HAVE AGREED TO HEREIN.
I warrant that I am at least eighteen years of age (or, if not, that my parent or legal guardian has agreed to the foregoing and signed below) and that I have the full and unrestricted authority to enter into a contract for myself (or, if not, that my parent or legal guardian has agreed to the foregoing and signed below).
If any provision of this agreement is held to be unenforceable, the remainder of it shall continue to be in full force and effect.
This waiver shall remain in effect until such time as I advise JFE otherwise in writing.
